How we verify
Nursing Home Inspections is compiled from public records, not from who paid to be listed.
Sources
- CMS Nursing Home Provider Information — Every Medicare/Medicaid-certified nursing home: star ratings, beds, ownership, chain, staffing hours and turnover, abuse icon and Special Focus Facility status.
- CMS Nursing Home Health Deficiencies — Every health citation from the last three standard inspection cycles plus complaint and infection-control inspections, with scope and severity codes.
- CMS Nursing Home Penalties — Civil money penalties (fines) and denials of payment for new admissions imposed in the last three years.
We load three files that CMS publishes for Medicare Care Compare and join them on each home's CMS certification number: provider information (ratings, ownership, staffing), health deficiencies (419,000+ citations) and penalties (fines and payment denials). We publish every certified nursing home in the file.
Star ratings are CMS's own Five-Star Quality Rating System scores (overall, health inspection, staffing and quality measures). We do not adjust them. Citations are shown as CMS records them: inspection date, the requirement cited, and the scope-and-severity letter from A (isolated, potential for minimal harm) to L (widespread, immediate jeopardy). Fines are CMS's reported amounts, which may reflect reductions for waived appeals.
The abuse icon and Special Focus Facility labels appear only where CMS itself assigns them. We do not create our own warnings, and we do not rank homes by fines or citation counts.
Order within a city: highest CMS overall star rating first, then alphabetical. Featured homes appear in a separate, labelled box and never change that order.
What a badge means
Certified: the nursing home holds an active CMS certification number and appears in the current CMS Nursing Home Provider Information file. We only publish positive matches. A listing without a badge is not an accusation — most gaps are name variations between registries.
Freshness
Data was last refreshed on 2026-09-30. CMS refreshes these files monthly; we re-pull them weekly.
